Compressive Non-Toxic Intrathoracic Goiter in Patient with Contraindication to Surgical Treatment: Case Report

Introduction: Thyroidectomy is the treatment of choice for non-toxic compressive Multinodular Goiter (MNG). However, when surgery contraindicated, other therapeutic options should be evaluated. In this case report, non-surgical possibilities are reviewed, and results obtained with Radioiodine Therapy (RAI) after stimulating Recombinant Thyroid-Stimulating Hormone (rhTSH) described. Case report: A 92-year-old patient multiple comorbidities, MNG, symptomatic compression trachea esophagus. Accordingly, RAI was chosen due to high surgical risk. Initially, pre-treatment methimazole performed increase TSH, which interrupted drug hepatitis. Then, in post-stimulus a low dose rhTSH, significant early reduction goiter symptoms. Discussion: Although total thyroidectomy intrathoracic MNG; it does present risks, especially patients comorbidities. Thus, interest less invasive techniques increasing. Thermal radio-ablations exhibit satisfactory results, however still an inaccessible technique. effective option subsequent use rhTSH or hypothyroidism methimazole-induced. RhTSH can level radiation absorbed by gland, lead required I-131 dose. Conclusion: It possible mitigate symptoms improve quality life MNG without intervention. report we describe alternatives intervention show effectiveness safety along management MNG.
